This form has to be given to your health care provider in order for your records to be released to Geriatric Review. Once this is completed, Geriatric Review will be responsible in obtaining these records from your provider to save you any inconveniences. |

It can then be either faxed or mailed to Geriatric Review for our records. This form ensures that all your records are held in strictest confidence and will not be viewed, shared or discussed with anyone outside of Geriatric Review.
