How To Get Started - Patient Process

	Step1 Please contact us by completing the online contact form or contact us directly at: Ph: (239) 963-8726 Fax: (239) 963-8727
	Step 2 Download, print and complete the patient forms in the forms section below.
	Step 3 Request your medical records from your provider and send them to Geriatric Review, if you are unable to do so, then we will request your records for you. There is usually a fee from an outside agency to request medical records, in this case the patient will be responsible.
	Step 4 Leave the rest to Geriatric Review and depending on your package you will receive your report in 2 - 4 weeks.

Process for Geriatric Review

	If you only request the medication card we will only need a list of your medications you are taking including supplements and vitamins.
	Dr. Garry will organize and review your medical records and make sure they are all encompassing, including pertinent EKG’s, CAT Scans, MRI, Stress test, Bone Density, Mammogram and any other tests you think should be included.
	All this medical information will be put onto a key chain drive or media of your choice. This information will be in a PDF format with adobe reader on a USB. This information will be password protected with a password of your choice.
	The USB key chain with your medical record report and medication card will be mailed to you within 2-4 weeks.

Patient Forms

Download the release form that has to be printed out and signed. Medical Records Release Form Patient Intake Form (Health Questionnaire) This form has to be given to your health care provider in order for your records to be released to Geriatric Review. Once this is completed, Geriatric Review will be responsible in obtaining these records from your provider to save you any inconveniences.

It can then be either faxed or mailed to Geriatric Review for our records. This form ensures that all your records are held in strictest confidence and will not be viewed, shared or discussed with anyone outside of Geriatric Review.
