Medical Record Synopsis Review

Dr. Garry will review your medical records and prepare a detailed summary. Included in this report will be the following:

	A list of all your medical history (both past and present)
	A list of all major surgical procedures
	A list of all your current medications and dosages
	A complete social history that includes all relevant information (living wills, health care proxies)
	A thorough family history

Dr. Garry will obtain your medical records and compile a succinct report that summarizes your medical issues. He will have your latest EKG’s, CAT Scans, MRI, Stress test, Bone Density, Mammogram and any other tests you think should be included. These reports will be summarized in your synopsis, but full reports will be available as well. Geriatric Review will supply all this information to you on a CD. For a small additional fee, Geriatric Review will put this information on a Key Chain Drive that you can give to your doctor or emergency room physician in order to upload your information. All the information will be password protected to ensure your confidentiality.
